TO BECOME A GetOnlineDoctor patient, you need to do several things:

  • Download and fill out, the Questionnaire Forms and Fax Cover Sheet. All fields are required to be filled in.

    Please Double-Check all information for accuracy.

  • Photocopies of pertinent Medical Records of your current medical complaint. Medical records that are considered valid must be current and sufficient to adequately establish the diagnosis and justify a course of therapy.
  • Copy of your Photo ID.
  • Download and fill out the Fax Cover Sheet

    FAX ALL of this information to: 1-727-447-3497.

After Get Online Doctor has received ALL DOCUMENTATION, your doctor’s consultation will begin. We approve documentation all day and after normal business hours. Please check your email often.

Approval: You will receive an e-mail approving you for a doctor consultation and further instructions.

Not Approved: You will receive an e-mail stating why you were not approved.

The email will also include your doctor telephone consultation appointment. All times are Eastern Standard Times (EST). The doctor will contact you at the phone number you have listed on your Questionnaire Forms. Please make sure to keep this information current. We strongly recommend that you do not miss your assigned consultation time.

If you are a New Patient your credit card will be charged $125.00 after your consultation and only if our doctor can offer you treatment.

If you are a Returning Patient your credit card will be $85.00 after your consultation and only if our doctor can offer you treatment.

In situations where GetOnlineDoctor.com cannot offer you a treatment option, a full refund will be credited back to your credit card, minus a $30.00 administrative fee.
